Microsoft 365 apps: Forms

To access all apps, log in to your Office 365 account and click the app launcher in the toolbar.

Create and distribute surveys, quizzes and polls. As results are submitted, use the built-in analytics to evaluate your data. Export to Excel is also available.

Eligibility

  • Faculty, staff and currently-enrolled students have access to Forms.
  • Forms you create are no longer accessible when your ECU PirateID account is deactivated.
  • Forms created in Microsoft Teams are stored within the Team. Therefore the Form is still available after the owner leaves the Team.
  • You cannot transfer ownership of a Form to another user. However, you can create and share a copy of the Form.
  • Prior to conducting new research, survey distribution is subject to the University & Medical Center Institutional Review Board. You must meet UMCIRB criteria before conducting research.
  • If a form will be distributed to more than 200 ECU students, faculty, staff or alumni on university property and/or using university resources, prior approval from the Survey Review and Oversight Committee is required. You should submit a request using the online request portal.

  • Adhere to university policies and guidelines.

Survey Regulation
The Institutional Survey Administration REG01.30.01 (informally the Survey Regulation) regulates the administration of surveys at East Carolina University.

The Survey Review and Oversight Committee reviews and approves surveys administered to faculty, students, staff or alumni on university property and/or using university resources.


Terms of Use
Forms is a hosted cloud system. It is recommended that you, as the survey owner, back up your data regularly to University-approved storage such as Microsoft OneDrive.

Forms is NOT APPROVED for the collection or storage of institutional (HR & Financial), FERPAHIPAA, or sensitive data. DO NOT COLLECT institutional, FERPA, HIPAA, or sensitive data using Forms.

East Carolina University requires that all data collected or stored in Forms meet federal, state or industry requirements. As a research best practice, all sensitive or confidential research data should be de-identified prior to collection or use. Please follow UMCIRB research guidelines and contact the IT Help Desk at 252.328.9866/800.340.7081 to request an IT Security consultation if you have questions concerning appropriate protocols for the collection and storage of sensitive or confidential data.

In addition to the above regulations, YOU AGREE NOT TO USE DATA that includes, but is not limited to, the information below:

  • Social Security Numbers (SSN)
  • Credit card numbers
  • Debit card numbers
  • Driver’s license numbers
  • Personally identifiable patient information
  • Personally identifiable student information
  • Personnel information
  • Confidential legal data
  • Proprietary data that should not be shared with the public
  • Employer taxpayer identification numbers
  • State identification card numbers or passport numbers
  • Checking account numbers
  • Savings account numbers
  • Personal identification (PIN) codes
  • Digital signatures
  • Any other numbers or information that can be used to access a person’s financial resources
  • Biometric data
  • Fingerprints
  • Passwords
