Graduate School Academic Exceptions and Corrections Request

Graduate Program Directors should submit this form to request academic policy exceptions, corrections, changes or additions for graduate level students. Does not include medical or dental students.

Student Change Term Request

The Change Term portal is to be submitted by Program Directors and Faculty that have been notified by a graduate student that they would like to change their admission term. Please submit one request for each user

Tuition Award and/or Assistantship Exception Request

Graduate program directors (or a designee) should submit this form when requesting an exception to the graduate school funding policy for an assistantship or tuition support. Please limit each request to one student.
